They are ranked 22nd because, wait for it, they are 22nd and have 21 teams in front of them.
They only play a few internationals a year because they are in, wait for it, South Africa, and it costs lots of money to travel to the United states and Canada like they did last year, and it costs just as much to travel here and to Europe. They have stated before that they would rather spend the money they do have on the domestic game and when they have more money, they'll start touring more, they started touring again in 2008.
If you don't play international matches as often as others, how are you supposed to move up the rankings, dumbarse?

And yet the Springboks travel to Australia and New Zealand all the time. And they are supposed to be RL juggernauts, right?

Yep, just as in the USA and Italy, RL in SA has 2 comps fighting it out to run the game.

open age senior teams who play in the 3 Provincial League Comps - 16 in North West, 6 in KwaZulu Natal (Durban), 6 in West Cape ( Capetown) while Gauteng Province based on Johannesburg is a year or two away from a comp.
National Championships are held annually between the Provincial teams drawn from 3. above and the National Team (Lions) is selected from this for the I'nat Cup, Annual Aust Indigenous V Lions clash & visiting teams (AFL Academy, Geelong Grammar etc)

It's auskick taken to schools where kids have to play during PE, and they don't just count the kids that play, the count the whole school.
